Chronic Care Management at McCamey County Hospital District

If you are enrolled in Medicare and have two or more chronic conditions, like asthma, diabetes, high blood pressure, arthritis, or depression, our Chronic Care Management (CCM) program is for you!

We work with you to coordinate care among different providers and make sure your medications work safely with each other.

Our Chronic Care Management Program includes:

  • 24/7 access to a health care professional for urgent care needs

  • Personalized care planning and ongoing care and medication management

  • Care coordination among pharmacy, specialists, testing centers, home health care providers, and hospitals

  • 20-minute monthly visit covering care management

Common chronic conditions include:

  • Alzheimer’s disease and related dementia
  • Asthma
  • Arthritis (osteoarthritis and rheumatoid)
  • Cancer
  • Chronic obstructive pulmonary disease (COPD)
  • Depression
  • Diabetes
  • Ischemic heart disease (CAD)
  • Hypertension
  • Anemia
  • Hyperlipidemia
  • Osteoporosis
  • Chronic Kidney Disease

  • Hypothyroidism

Insurance

Depending on your insurance, you may be charged a copay. Typically, the amount is $8-9 per month. If you have coinsurance, supplemental insurance, or Medicaid, the copay may be covered. Our office will bill Medicare for the Chronic Care Management services once a month. Your account will reflect this charge and you will be responsible for any copay that is not covered.

Choose McCamey County Hospital District for your CCM

You can only be enrolled in a CCM program with one provider. We hope you choose McCamey County Hospital District to manage your CCM program. If you already signed an agreement with another provider or office, please let us know.
